  3. If youΓÇÖd rather pay by check, then choose Order by Mail/Fax. With this option, you will print out an order form on paper.  You can also fax your order if you want to pay by credit card.

  5. To print your order form select the Print Order Form button. When your print out is completed, select the Done button.
